Submit an anonymous safety concern

    Before you begin

    Role required:
    • For Employee Center: sn_ohs_im.requestor
    • For Anonymous Report Center: None/Anonymous reporter

    About this task

    The Anonymous report center [com.sn_anonymous_report_center] plugin must be installed to enable reporting a safety concern anonymously. For more information, see Activate the Anonymous Report Center.

    Procedure

    1. Navigate to All > Self service > Employee Center.
    2. In the Health and Safety tab
      • Select the Anonymously report a safety concern quick link.
      • Select Health and safety observations and then select the Anonymously report a safety concern card.
      • Select the Browse health and safety button and then select the Anonymously report a safety concern card.
    3. Select Continue on the modal dialogue.
      You are signed out of the Employee Center and signed into the Anonymous Report Center as a guest user.
    4. Select the Submit an Anonymous Report card and then select the Health and safety concern card.
    5. On the form, fill in the details.

      Field Description
      Do your best to describe the situation Information that accurately describes the safety concern that you witnessed with as much detail and your recollection of the events as possible.
      Note:
      All information provided is confidential.
      List the people who are involved and how they are involved The names of the individuals involved in the safety incident. Include any witnesses to the incident.
      Note:
      Full names, job titles, teams, and departments are valuable for the investigators.
      Provide the date and time relevant to the situation The date and time of the safety concern.
      Note:
      Estimated dates are acceptable.
      Let us know where this situation is taking/took place The name of the location where the safety concern occured.
      Note:
      Details like room, desk spaces, or areas within the building the safety concern took place are all helpful to investigators.
      I understand that the information I'm providing, including any sensitive or personal details, will be reviewed by the appropriate agents and teams. After completing the form, check the box.
    6. Enter the verification code and select Submit.
    7. Select Download report copy to save a PDF copy of your record.
      The report key and report number are required to:
      • Access updates or comments related to your anonymous report.
      • Track, ask questions, and view or add details about the anonymous case.
      • Respond to any follow-up questions.
      Note:
      The anonymous report can’t be accessed without the report key.

    Result

    The anonymous report is submitted and you can use the report key and report number to check the progress of the report.
