  • Safety managers and safety agents can use the Health and Safety incident form to create an incident record or to work on a safety incident.

    Table 1. Enter incident summary form
    Field Description
    Number System-generated unique number for the incident.
    Occurred on Date and time of the incident, displayed in the viewer's local time zone.
    Occurred on (local) Date and time of the incident, displayed in the time zone of the user who reported it.
    Sensitive content Option that indicates that the incident contains sensitive information.

    Only a user with the Sensitive content user role can view a report marked as sensitive. For more information on roles, see Components installed with Health and Safety Incident Management.

    Note:
    If you select this option without the sensitive content user role while creating the record, you will not be able to view the record after saving.
    Watch list Users notified of updates to the incident record.
    Assignment group User group responsible for managing this incident.
    Category Classification of the incident type.
    State Status of the incident.
    Severity Level of impact or seriousness of the incident.
    Service Service associated with the incident.
    Opened by User who created the incident record.
    Assigned to User assigned to this case.
    Closed in the field Option that indicates the incident was resolved on-site.
    Short description Brief description of the incident
