Chemical item form
The chemical manager uses the chemical item form to add a new chemical item record. The field and its descriptions are as follows.

| Storage condition | Condition in which the chemical is stored. |
| Max allowed quantity | Maximum quantity allowed to be stored. |
| Issue date | Date the chemical will be issued. |
| Quantity | Quantity of the chemical. |
| Review date | Date of next chemical review |
| Date last reviewed | Date the chemical was last reviewed. |
| Expiry date | Date after which the chemical must not be used. |
| Additional comments | Additional comments relevant to the chemical. |
