Create Universal Request for Health and Safety cases as an employee

  • You can submit a Universal Request for Health and Safety cases from the Employee Center using the Request Help form.

    Before you begin

    Role required: none

    Procedure

    1. Navigate to All > Self-Service > Employee Center.
    2. Search for help and select the Request Help card.
    3. On the Request Help form, fill in the details.

      How do you summarize your issue Summary of the issue
      Anything else we should know More information about the issue
      Issue contains sensitive or confidential information Check box to tag the issue as confidential
      Select department for your issue

      Name of the department the issue is related to

      This field appears when the Issue contains sensitive or confidential information field is selected

      Add attachments Option to attach documents if any
    4. In the Select department for your issue field, select Health and Safety.
    5. Select Submit.

    Result

    • The request is transferred to the Health and Safety department.
    • The universal request number from which the health and safety case is created appears in the Universal Request field on the health and safety case form.
    • The health and safety case number appears in the Primary Ticket field on the universal request form. It also appears under the Associated Tickets related item.
