    Field Description
    Employee profile Employee profile associated to the healthcare professional.
    Professional profile Professional profile associated to the healthcare professional.
    Employment history type Option indicating whether employed or had a gap in employment.
    Employment type Nature of employment, for example, Consultant, Trainee, or Contractor.
    Start date Date when the employment began at an organization.
    End date Date when the employment ended at an organization.
    Status Status of the employment history record:
    • New: When profile intake form is sent to the healthcare professional for filling.
    • Pending validation: When data is filled in by healthcare professional and sent for credentialing and validation to HR agents.
    • Invalid: When HR agents review the data and reject due to insufficient or invalid details.
    • Valid: When HR agents validate and approve the information provided by the healthcare professional.
    Employer name Name of the organization at which the healthcare professional has worked.
    Employer address Address of the organization at which the healthcare professional has worked.
    Employer city City in which the employer office is located.
    Employer state/province State in which the employer office is located.
    Employer country Country in which the employer office is located.
    Employer zip/postal code Postal code of area in which the employer office is located.
    Gap reason Reason stated by the healthcare professional when Employment gap is selected in the Employment type field.
