Use the Gift Policies form to add policy in accordance with your organization's policy.

| Select the category for gifts or entertainment | Category of the transaction. Note: You can customize the options by adding subcategory. For more information, see [Add a subcategory for the intake form](https://servicenow-prod.fluidtopics.net/ASHxCUGHEvsk8nyegmfQIA "Add a subcategory to an intake form of a practice area to organize legal requests and assignments through the intake form."). |
| Provide additional information | Additional information when the category selected in Others. |
| Are you giving or receiving any gifts or entertainment? | The type of transaction. * Giving: When the requester is giving a gift or entertainment. * Receiving: When the requester is receiving a gift or entertainment. {#lsd-ge-disclosure-form__ul_rm1_frk_tcc} |
| Select the recipient - is it someone within the company or an external party? | Recipient that is either internal or external to the organization. * Internal organization: An employee within the company who is giving or receiving gifts or entertainment. * External party: A user outside of the company who is giving or receiving gifts or entertainment. {#lsd-ge-disclosure-form__ul_okn_1sk_tcc} |
| Provide the approximate event date | Date of the transaction. |
| Provide the approximate value for the gift or entertainment | Value of the gift or entertainment. |

| Company personnel accepting or offering gifts and entertainment | Name of the company personnel giving or receiving. |
| A government entity or government personnel? | Indicates whether the disclosure involves a government entity or personnel. |
| A relative of government personnel? | Indicates whether the disclosure involves a relative of government personnel. |
| Affiliated with a state-owned enterprise (such as a public-funded university or hospital)? | Indicates whether the company involved is state-owned. |
