Create a permit type to be selected in the permit-to-work request form.

## Before you begin

Role required: admin and sn_hs_rm.permit_coordinator

## About this task

The admins must assign specific roles to the sn_hs_rm.permit_coordinator role to enable the permit coordinator to perform this configuration. For more information, see [Configure permit-to-work](https://servicenow-prod.fluidtopics.net/tnKDo6FLtUWBq8kInECS6Q "Configure permit type, permit checklist, checklist type, and edit a permit form.").

## About this task

Selecting a permit type in the form automatically adds related questions specific to that permit type.

## Procedure

1. Navigate to AllSystem definitionTables and select the Permits to work table.
2. Select Permit type in the Columns tab of the related list.
Opens the Dictionary entry for permit type.

4. On the form, fill in the fields.
For more information on the field description, see [Choice new record form](https://servicenow-prod.fluidtopics.net/4LOQQmOpeCl73qihaB9P2A "Field description to add a new choice to create a new permit type.").
5. Select Submit.
A new permit type is created which appears in the Permit type field of the permit-to-work request form. Repeat the steps to create multiple permit types.
