# Add an action to a request

# Add an action to a health and safety permit-to-work request {#ariaid-title1}

Create a Health and Safety action to a requested permit-to-work. This helps to address the root cause of any reported safety occurrence.

## Before you begin

Role required: sn_hs_rm.permit_coordinator

## Procedure

1. Navigate to AllHealth and SafetyHealth and Safety Workspace.
2. Select the risk management icon (![Risk assessment icon]()).
3. Select the Permit to work list and select All.
4. Open a permit record to add an action.
5. In the Health and Safety actions tab of the related items, select New.
6. On the form, fill in the fields.  
   For information on field description, see [Health and Safety action form](https://servicenow-prod.fluidtopics.net/ikF88VSFopU3QEqywJaAlg "Safety agents can use the Health and Safety action form to assign an action for a safety incident or observation.").
7. Select Save.  
   The action appears in the list of actions in the **Health and Safety actions** tab of the permit record. It is also listed in the **Actions** list on Health and Safety Workspace.
