Conduct a safety risk assessment to identify and manage potential hazards and risks, and to ensure the safety of your employees and the public.

## Before you begin

Role required: sn_hs_rm.risk_assessment_manager or sn_hs_rm.risk_assessment_writer

## Procedure

1. Navigate to AllHealth and SafetyHealth and Safety Workspace.
2. Select the risk management icon (![Risk assessment icon]()).
3. Select the Risk assessment list and then All.
4. Select New to start a risk assessment.
5. On the form, fill in the fields.
Enter details such as who is at risk, who is responsible for controls, who reviews, and who approves this risk assessment?

For more information on risk field descriptions, see [Risk assessment form](https://servicenow-prod.fluidtopics.net/DFq~k3GFNGSovap_pqgGTg "Risk assessment managers can use the risk assessment form to conduct their risk assessments.").
6. **Optional:** If necessary, attach an image about the risk using the add attachments icon (![Add attachment icon.]()).
7. Select Save.
The Risks and Controls and Risk actions tabs appear for this risk assessment.
8. List any identified hazards and choose their controls in the Risks and Controls tab.
Create a Risk and Controls record for each identified hazard. For more information, see [Add a risk and its control measures for a safety risk assessment](https://servicenow-prod.fluidtopics.net/DKXxOoRLIiG9Qge4HhKt9Q "For a risk assessment, add an identified risk and choose control measures to assess how they impact and mitigate the risk.").
9. If applicable, assign actions in the Risk actions tab.
For more information, see [Add an action for a safety risk](https://servicenow-prod.fluidtopics.net/64OEaF5PUjvS_NiWhvpkCw "Add an action to address an identified risk or implement control measures necessary to improve workplace safety. You can add multiple actions for a hazard or control measure.").

## Result

The overall level of risk, both before and after controls, is based on the severity and likelihood of each hazard that is listed in the Risks and Controls tab. It is determined based on the highest
identified risk from the risks and controls list. The computed risk levels appear in the Risk before controls and Risk after controls fields in the risk assessment's form
header.

## What to do next

After the risk assessment is complete, assign a reviewer and approver for it, if necessary. Only a user with the Risk assessment approver \[sn_hs_rm.risk_assessment_approver\] role can be assigned as an approver.
