  • Employees can use the Ethics Complaints intake form to report an ethics violation incident.

    Field Description
    Complaint type Type of complaint or allegation that violated the ethics policy of the organisation.
    Detailed description Detailed information about the incident, such as where and when it occurred.
    Involved parties Users who are the subject of the complaint.
    Provide the approximate date and time of the incident Tentative date when the incident occurred.
    Add involved parties from the Add Row dialog box that appears by clicking the Add button in the Involved Parties section of the Ethics Complaints form.

    Field Description
    Involved party type Type of involved party.
    • Internal: An employee within the organisation who is the subject of the complaint.
    • External: A user outside of the organisation who is the subject of the complaint.
    Internal party name Name of the involved party.

    The Internal party name field is replaced by the External party name field when External is selected as the involved party type.

    Email Email ID of the involved party.

    For the internal employee, the email ID is auto-populated from the User profile.

    For an external employee, enter the email ID. This field is available only when External is selected from Involved party type.

    Notes Any additional information about the involved party.
