Finding form for safety audits
When an audit's item for review is complete, the safety auditor can use the Findings form to log findings gathered from the review or survey responses.

| Number | System-generated unique number for the finding. |
| Short description | Brief description about the finding. |
| Hazard | Potential hazard identified during the audit. |
| Risk assessment | Risk assessment for identified hazard. |
| Location | Location that this finding applies to. |
| Assigned to | User this finding is assigned to. |
| Description | Detailed description about the finding. |
| Attachment | Supporting files related to this finding. Select Attach File to upload the file. |

| Item for review | Item for review from this audit that this finding is relates to. Only the review items associated with objectives in this audit appear in this field. For more information, see Define an objective for a safety audit. |
| Survey | Audit survey or questionnaire that includes this finding as a response. This field appears and is auto-populated when an audit survey is selected from Item for review. |
| Section | Category of survey questions. This field appears only when an audit survey is selected from Item for review. The field lists all question categories from the selected survey. |
| Question | Survey question this finding is submitted for. This field appears only when a category is selected from Section. The field lists all questions in the selected survey category. |
| Question Response | Submitted answer for the survey question. This field appears and is auto-populated with the response submitted for the selected survey question. |
| Criteria type | Criteria type this review item belongs to. |
| Criteria | Standards that the audit measures compliance against. The items in this list appear based on the selected Criteria type and the selected record of that type in audit's objectives. For more information, see Define an objective and criteria for an audit. |
| Criteria document | Link to the specific citation or regulation within the selected criteria record. This field appears only when Regulatory list or Authority document is selected from Criteria type. |
| Cited criterion | Specific clause, paragraph, section, or page reference entered from the selected criteria. This field appears only when H&S Document, Knowledge base article, or Text is selected from Criteria type. |
| Compliance | Compliance status of the system covered in the audit survey. |
| Work notes | Additional details about the finding. |
