Use Listening Posts with Journey Accelerator to add surveys to action plans and to-do tasks to enable immediate feedback based on the context of a task.

Administrators can create templates to trigger surveys using Listening Posts so that a survey is opened when a task is completed.

Activate the Listening Posts (sn_lp) plugin from ServiceNow Store. Listening Posts is available with the HR Enterprise package.
