Finding form
When an inspection or audit survey is complete, the safety manager can use the Findings form to log findings gathered from survey responses.

| Number | System-generated unique number for the finding. |
| Short description | Brief description about the finding. |
| Location | Location that this finding applies to. |
| Hazard | Potential hazard identified during the survey. |
| Risk rating | Risk rating for identified hazard. |

| Survey | Audit survey that includes this finding as a survey response. |
| Section | Category of survey questions. This field appears only when a survey is selected from Audit survey. The field lists all question categories from the selected survey. |
| Question | Survey question this finding was submitted for. This field appears only when a category is selected from Audit survey category. The field lists all questions in the selected survey category. |
| Question Response | Submitted answer for the survey question. This field is auto-populated with the response submitted for the selected survey question in the survey. |
| Compliance | Compliance status of the system covered in the audit survey. |
| Work notes | Additional details for the finding. |
