    • Request non-medical device issue biomed service using the Request other biomed service catalog item from the Care Team Portal.

    Role required: sn_hco.care_team_member

    Procedure

    1. In the Care Team Portal, navigate to Create a request > Request biomed service > Request other biomed service.
    2. Fill in the following fields.
      Field Description
      Unit The unit associated with the biomed issue.
      Location The location of the biomed issue.
      Short description

      A short summary of your issue.
      Describe your request in detail

      Add a detailed description of the biomed issue that has occurred.

      Provide as much relevant information as possible.
      Urgency The urgency of this issue. Options are:
      • 1 - High
      • 2 - Medium
      • 3 - Low
      Contact person The associated contact for this request.
      Phone type The phone type associated with the contact number. Options are:
      • Business
      • Mobile
      • Other
      Contact number The phone number of the associated contact for this request.
    3. Select Submit.
