Report a medical device issue with Care Team Operations for Biomed
Report any issue with a medical device issue using the Report a medical device issue catalog item from the Care Team Operations portal.

Procedure
- In the Care Team Portal, navigate to Create a request > Request biomed service > Report a medical device issue.

Fill in the following fields.
Field Description Unit The unit associated with this request. Asset tag or serial number The asset tag or serial numeric value associated with the medical device experiencing an issue. Identified asset Asset model The asset model of the medical device experiencing an issue. Location The location of the medical device. Short description A short summary of your issue.
Describe your request in detail Add a detailed description of the device issue occurring.
Provide as much relevant information as possible.
Urgency The urgency of this issue. Options are: - 1 - High
- 2 - Medium
- 3 - Low
Contact person The associated contact for this request. Phone type The phone type associated with the contact number. Options are: - Business
- Mobile
- Other
Contact number The phone number of the associated contact for this request. - Select Submit.
