Request other IT service with Care Team Operations for IT

  • Request IT service for non-EMR issues using the Request other IT service catalog item from the Care Team Portal.

    Role required: sn_hco.care_team_member

    Procedure

    1. In the Care Team Portal, navigate to Create a request > Request IT service > Request other IT service.
    2. Fill in the following fields.
      Field Description
      Unit The unit associated with this IT request.
      Asset model The asset model of the device experiencing an IT issue.
      Asset tag or serial number The asset tag or serial numeric value associated with the device experiencing an IT issue.
      Identified asset
      Short description

      A short summary of your issue.

      Describe your request in detail

      Add a detailed description of the IT issue occurring.

      Provide as much relevant information as possible.

      Urgency The urgency of this issue. Options are:
      • 1 - High
      • 2 - Medium
      • 3 - Low
      Contact person The associated contact for this request.
      Phone type The phone type associated with the contact number. Options are:
      • Business
      • Mobile
      • Other
      Contact number The phone number of the associated contact for this request.
    3. Select Submit.
