Use this form when creating a new evidence record within an Investigative Case Management case.

| Type | Type of evidence. |
| Evidence location (Physical) | If physical evidence, the physical location of the evidence. |
| Evidence location (Digital) | If digital evidence, the digital location of the evidence. |
| Evidence related notes | Any additional notes about the evidence. |
| Originating Case | The case that originated this evidence, and the case that the evidence record is linked to. |
| Originating Case ID | The id of the case that the evidence record is linked to. |
| Security classification | Determines access to evidence based on user or team role. |
| Restrict access? | Whether to restrict access to certain roles. Yes/No. |
| Collection Location | Where the evidence was collected from. |
| Collected By | The individual who collected the evidence. |
| Collection Date | The date on which the evidence was collected. |
| Collection Time | The time in which the evidence was collected. |
| Source From | Whether the evidence record is from an investigative task, a related case, or other. |
