Select New to create a template category.

In the Category role field, enter sn_smart_asmt.template_manager.

Select New Template, and on the form, fill in the fields.
In the Assessment targets field, enter Grants Management Case.

Navigate to the Questions tab and select Add section.

On the form, fill in the details for the section, then select Save.

Select Add question, and add a Single Select Drop-down Question of your choice.
Repeat until you have added all of your desired questions.

Select Publish.
