Applicant organization name	The legal name of the applicant.
Registered address line 1	The first line of the applicant's registered address.
Registered address line 2	The second line of the applicant's registered address.
Registered address line 3	The third line of the applicant's registered address.
Applicant Postal code	The postal or ZIP code for the applicant's registered address.
Country	The country of the applicant's registered address.
Legal status	The legal status of the applicant, for example, individual, corporation, and so on.
Country of company registration	The country of the applicant's company registration or private residence.
Corporate registration number	The corporate registration number or passport number of the applicant.
VAT number	The value added tax (VAT) number of the applicant.
URL	The URL of the applicant's website.
Main business	The main business of the applicant, for example, food retailer.
Year of registration	The year that the applicant's business was registered.
Number of employees	The approximate number of employees in the applicant's business.
CEO	The name of the Chief Executive Officer or General Manager of the applicant's business.
CEO passport number	The passport number of the Chief Executive Officer or General Manager of the applicant's business.
Countries of operation	The countries in which the applicant's business is active.
Main shareholders	The main shareholders of the applicant's business (>10% ownership).
Balance sheet	The balance sheet total for the applicant's business for the last financial year in USD.
Email address accounts department	The email address for the accounts department of the applicant's business.
Contact Details
Organization name	The name of the organization.
Address line 1	The first line of the organization's address.
Address line 2	The second line of the organization's address.
Address line 3	The third line of the organization's address.
Postal code	The postal code of the organization's address.
Country	The country in which the organization is located.
Contact person	The person who is the primary contact for the organization.
Email	The email address for the organization.
Telephone	The telephone number for the organization.
Fax	The fax number for the organization.
IREC orgs	Any existing I-REC registry organizations that will become subsidiaries of the organization.

Family name surname	Last name of the lead user's family.
