| Support group | SRM team managing the service. |
| Business criticality | How important this service is the business. |
| Owned by | Service owner. May not be part of an SRM team. |
| Operational Status | Status for the service's ability to perform operational functions. |
| Stakeholders | Product owners or service consumers who determine the target availability of a service. |
| Environment | Choices are: * None * Development * Production * Test {#sr-edit-service-form__ul_y3s_jfk_mbc} |
| Managed by group | Team manager responsible for the service. Add by search or edit using the text editor. |
| Change group | Team responsible for changes. Add by search or edit using the text editor. |
| Status | Status of the service. |
| Life Cycle Stage | Choices are: * Deploy * Design * End of Life * Ideation * Operational {#sr-edit-service-form__ul_zzx_mfk_mbc} |
| Phase | Phase of the service. Choices are: * None * Pipeline * Catalog * Retired {#sr-edit-service-form__ul_ldy_xhk_mbc} |
