| Short description | Brief description of the request. For example, legal_entity_template. |
| Type | Type of the download request. The choices are as follows: * Excel master template * Plain-CSV reporting package * Assessments * Branches * Contracts * Functions * Legal entities * Supply chains * Third parties * Third-party engagements * Third-party information register {#create-excel-upload-download-req__ul_r2y_ckr_xcc} |
| Number | Number of the request, for example, REQ0001001. This field is auto-filled. |
| Request type | Type of the request. The choices are as follows: * Upload * Download {#create-excel-upload-download-req__ul_cvn_3bd_zvb} |
| State | State of the record. This field is auto-filled to Draft state. |
| Base currency | Currency used for reporting monetary values in the report. For example, US Dollar or Euro. |
| Report type | Specifies whether the report covers a single entity Individual or a Consolidated group of entities. |
