# Configure a form header

# Configure a form header for a workspace {#ariaid-title1}

Create a form header and add it to a workspace's form configuration.

Role required: workspace_admin, ui_builder_admin, admin

After creating a form header, you must add it to your workspace's form header configuration.

One form header configuration is included with each workspace. You can use this configuration without completing any additional steps.

## Procedimento

1. Create a form header.
   1. Navigate to AllNow Experience FrameworkConfiguration SettingsUX Form Header.
   2. Select New.  
      A new Form Header record opens.
   3. Complete the following fields:  

      Table
      :   Select the table where the form header will apply.

      Primary field
      :   Select a primary field for the form header.

      Subheading
      :   Select a subheading for the form header.

      Header Image
      :   Select a header image for the form header.
   4. Select Submit.

2. Add the form header to the form header configuration.
   1. Navigate to AllNow Experience FrameworkConfiguration SettingsUX Header Configurations.
   2. Select the header configuration for your workspace.  
      The UX Header Configuration record opens.
   3. From the Workspace Form Headers related list, select Edit.  
      Nota:  
      You may need to configure the form to display the Workspace Form Headers related list.
   4. Add the form header.
   5. Select OK.
