# Add decisions for a project

As a project manager, develop a plan to manage risks and issues proactively with
solutions. Add the solution for a risk or issue to a project in the form of a decision. You
can also convert a risk or an issue to a decision or a decision to an action or a request
change.

## Antes de Iniciar

Role required: it_project_manager

## Procedimento

1. Navigate to ProjectProjectsAll.
2. Select the project to which you want to add a decision.
3. In the Decisions related list, select New.
4. On the Decision form, fill in the fields.  
   For more information, see [Risk, issues, decisions, actions, and request change (RIDAC) form](https://servicenow-prod.fluidtopics.net/o3gK1G8zm46BCHtQLNo_pA "Learn about the fields of the RIDAC form.").
5. **Opcional:** Search in the knowledge base for any article related to the decision.
   1. Select the search knowledge icon (![Search knowledge icon]()).
   2. If you find relevant articles, select the title of an article to view its content.
   3. If you want to include the content of the article in the issue, select Attach to Decision.  
      The article content is copied in to the Description field of the Decision form. You can modify the text if necessary.

6. Select Submit.

* Associate the decision with your existing issues and risks so you can track dependencies and recognize trends for future.
