Add actions that are required for resolving an issue or risk for a project. You can
also convert a risk, issue, or decision to an action based on your analysis and plan for
resolution of a risk or issue.

## Por Que e Quando Desempenhar Esta Tarefa

After analyzing the risks and issues and taking a decision on how to manage those
risks and issues, add an action for resolving the risk or issue to a project.

## Procedimento

1. Navigate to AllProjectProjectsAll.
2. Select the project to which you want to add an action.
3. From the Actions related list, select New.
4. On the Action form, fill in the fields.  
   For more information, see [Risk, issues, decisions, actions, and request change (RIDAC) form](https://servicenow-prod.fluidtopics.net/o3gK1G8zm46BCHtQLNo_pA "Learn about the fields of the RIDAC form.").
5. **Opcional:** If the action requires approval from other stakeholders, request approval with a due date using the Approval and Due date fields.
6. Select Submit.
