Use the employee form to create a new employee form record for Universal Task

| Name | Name of the employee form. |
| Description | Description of the employee form. |
| Parent table | Parent table available for the employee form. Parent table is a department-specific table associated to a universal task. For more information configuring a parent table, see [Configure Universal Task for your service](https://servicenow-prod.fluidtopics.net/NUyV52zsji620mrH2x~0RA "Configure the Universal Task application for your service so that you can create and assign tasks to employees."). Nota: * Only the tables that are configured for Universal Task with Collect Employee Input task type can be selected. * You can select a parent table only if you have write access to it. {#ut-emp-form__ul_lh3_lvb_51c} |
