# Request a vaccine exemption

Request a vaccine exemption from your organization with information about why you
require an exemption.

## Antes de Iniciar

Vaccine exemptions must be enabled by your organization to submit vaccine exemption
requests. If you do not see the Request vaccine exemption module or you are unable
to submit vaccine exemption requests, refer to your administrator to enable this
feature.

Role required: none

## Procedimento

1. Navigate to AllVaccination StatusRequest vaccine exemption.  
   Nota:  
   To request a vaccine exemption in the Now Mobile app, navigate to HealthFollow These Safety PracticesRequest Vaccine Exemption.
2. On the form, fill in the fields.

   |-|-|
   | What is the vaccine exemption type? | The type of exemption you are requesting. Available values are religious, medical, or other. |
   | Details for vaccination exemption request | Provide any useful additional details about the request for your organization. |

3. Complete the vaccine exemption form.
   1. Select a value for What is the vaccine exemption type?, such as Other, Religious, or Medical.
   2. **Opcional:** Provide the necessary details for the vaccine exemption request.

4. **Opcional:** Add any relevant attachments to the form.
5. Click Submit.
6. **Opcional:** Review the status of your vaccine exemption request by navigating to HealthFollow These Safety PracticesVaccine Exemption Requests in the Now Mobile app.

## Resultado

Your vaccine exemption request is submitted to your organization for an administrator or a manager to review.
