| Is this for a Vendor, Customer, or Partner? | Third party type with whom the contract has to be signed. |
| Purpose of the contract | Subject and purpose of the contract. |
| Contract type | Type of contract. Only active contract types are listed in the field. For more information, see [Create a contract type](https://servicenow-prod.fluidtopics.net/kaisAGZfX1LQddwpObJJXg "Create a contract type for defining different types of legal contract requests."). |
| Upload contract document | Contract document to be reviewed by the legal department. |
| Company legal name | Legal name of the third party with whom the contract has to be signed. |
| Expected completion date | Date by which the contract review has to be completed. |
| Other party legal point of contact name | Name of the person from the third party who is the point of contact. |
| Other party legal point of contact email | Email address of the point of contact. |
