| Date of occurrence | Date when the malpractice occurred. |
| Date of claim filing | Date when the malpractice claim was submitted. |
| Profession liability insurance involved | Insurer selected for the claim. |
| Description | Additional details about the malpractice claim. |
| Active | Option indicating that the malpractice claim history record is active. |
| Is in NPDB | Option indicating that the claim history is in the National Practitioner Data Bank (NPDB) database. |
| Status | Status of the malpractice claim history record: * New: When profile intake form is sent to the healthcare professional for filling. * Pending validation: When data is filled in by healthcare professional and sent for credentialing and validation to HR agents. * Invalid: When HR agents review the data and reject due to insufficient or invalid details. * Valid: When HR agents validate and approve the information provided by the healthcare professional. {#hr-mal-his__ul_cnc_gxx_pbc} |
| Claim status | Current status, whether the claim is in progress or resolved. |
| Resolution method | Type of resolution provided for the claim, for example, settled with prejudice, mediation, or arbitration. |
