The chemical manager uses the chemical item form to add a new chemical item record. The field and its descriptions are as follows.

| Description | Detailed description of the review. |
| Contact details ||
| Company | Name of the company that has the chemical item. |
| Responsible group | The group responsible for the chemical item. |
| Contact person | Name of the person to be contacted for emergencies regarding the chemical item. |
| Responsible person | Name of the person responsible for the chemical item. |
| Chemical information ||
| Storage condition | Condition in which the chemical is stored. |
| Max allowed quantity | Maximum quantity allowed to be stored. |
| Issue date | Date the chemical will be issued. |
| Quantity | Quantity of the chemical. |
| Review date | Date of next chemical review |
| Date last reviewed | Date the chemical was last reviewed. |
| Expiry date | Date after which the chemical must not be used. |
| Additional comments | Additional comments relevant to the chemical. |
| Work notes | Any work notes relevant to the chemical. |
