Role required: sn_hs_rm.risk_assessment_manager or sn_hs_rm.risk_assessment_writer

## Procedimento

1. Navigate to WorkspacesHealth and Safety Workspace.
2. Select the risk management icon (![Risk assessment icon]()).
3. Select the Risk assessment list and then All.
4. Select New to start a risk assessment.
5. On the form, fill in the fields.
Enter details such as who is at risk, who is responsible for controls, who reviews, and who approves this risk assessment.

For more information on risk field descriptions, see [Risk assessment form](https://servicenow-prod.fluidtopics.net/dgGDLeOA0S8g025GgHBEGw "Risk assessment managers can use the risk assessment form to conduct their risk assessments.").
6. **Opcional:** If necessary, attach an image about the risk using the add attachments icon (![Add attachment icon.]()).
7. **Opcional:** Research content related to the risk assessment by selecting the Agent assist icon (![agent assist icon]()).
Agent assist searches the knowledge base using the text from the risk assessment's Short description field and shows relevant articles.
8. Select Save.
The Risk analyses, Risk actions, and Action schedules tabs appear for this risk assessment.
9. In the Risk analyses tab, add risk analysis records to evaluate the risk level of identified hazards.
For more information, see [Perform risk analysis for a safety risk assessment](https://servicenow-prod.fluidtopics.net/Std~L30S1oyLPs7AoyCRPA "For a risk assessment, perform risk analyses of identified hazards and choose control measures to assess how they impact and mitigate the hazard.") and [Create a risk analysis from a job](https://servicenow-prod.fluidtopics.net/DlMe3yDAhhfLTgTfUdXHWg "Create a risk analysis by importing hazards and controls from a predefined job in the job register to a risk assessment.").
10. In the Action schedules tab, set up actions to address an identified risk or implement control measures necessary to improve workplace safety.
The action schedule creates actions in the Risk actions tab and auto-assigns them to individuals using the condition builder. For more information, see [Create a Health and Safety action schedule](https://servicenow-prod.fluidtopics.net/dTQ6uKAHPJYcrl125qvNvA "Create an action schedule to schedule actions across the organization such as learning activities and recurring tasks.").
11. In the Details tab, mark the risk assessment as complete when all associated risk analyses are finished.
    1. In the Assessment completed field, select the completion date.
    2. Set the State field to Complete.

## Resultado

* The overall level of risk, both before and after controls, is based on the severity and likelihood of each hazard that is listed in the Risk analyses tab. It is determined based on the highest identified risk in the Risk analyses list.
* The computed risk levels appear in the Inherent risk and Residual risk fields in the risk assessment's form header.
