| Version | Version number of the app you're reviewing. | Not publicly visible. |
| First Name | First or given name. | Not publicly visible. |
| Last Name | Last or family name. | Not publicly visible. |
| Email | Your email address. | Not publicly visible. |
| Company | Name of your company or organization. | Publicly visible for commercial customers. Replaced with "Verified ServiceNow Customer" for customers using a regulated ServiceNow Store instance. |
| Review | Feedback about the app. Your feedback could include whether you recommend the app, how user-friendly it is, features you'd like to see added in the future, and so on. | Publicly visible. |
