The pre-authorization form enables you to enter the pre-authorization details.

| Field | Description |
|-|-|
| Asset | Select the asset. |
| Requested by | Select the person who raised the request. |
| Consumer | Select the name of the consumer. |
| Product usage | Enter the number of Asset used. It provides insights into how customers use a product. |
| Description | Brief description about the pre-authorization request. |
| Dealer | Select the dealer. |
| Reported date | Select the date on which pre-authorization claim required is submitted. |
| External repair order number | Entered by the dealer service advisor when initiating a claim for a completed repair. |
| Unit of measure | Quantity of assets that are raised for pre-authorization claim. Unit of measure is configurable. Available options are: * Box * Bundle * Carton * Case * Days * Each * Kit * Month * Pack * Year {#mco-pre-auth-form__ul_tp4_b5f_sfc} |
