The DEX Self-service device actions form presents elaborate data on the form's fields and their corresponding descriptions.

| Quick action label |
The label displayed to users when the action appears in Employee Center, Now Assist, or the DEX Desktop Assistant. |
| Application |
The application scope this device action belongs to. |
| Active |
Indicates whether the device action is available to users. When the check box is cleared, the action is not visible on any interface regardless of the issue configuration settings. |
