The request change form enables you to request for a change in the demand.

| Field | Description |

| Number | System-generated ID number with a configurable prefix. |
| State | Current state of the change request. Change requests are created in the Open state. The available values are: * Pending * Open * Work in Progress * Closed Complete * Closed Incomplete * Closed Skipped {#create-request-change-form-ppw__ul_xsq_knk_bvb} |
| Priority | Urgency for approving the requested changes based on impact. Change requests are created with Low priority. The available values are: * Critical * High * Moderate * Low * Planning {#create-request-change-form-ppw__ul_ttg_rwx_tjc} |
| Estimated Cost | Estimated cost generated by the requested changes. |
| Impact | Impact of the change on the demand. Change requests are created with Low impact. The available values are: * High * Medium * Low {#create-request-change-form-ppw__ul_jwr_5wx_tjc} |
| Approval | Status of approval from the stakeholders. The available values are: * Not Yet Requested * Requested * Approved * Rejected {#create-request-change-form-ppw__ul_avr_d2p_c3c} |
| Assigned to | Resource assigned to work on the change request. |
| Due date | Requested date to complete the change request. |
| Category | Entity associated with the change request. The available values are: * Resource * Scope * Cost * Schedule {#create-request-change-form-ppw__ul_uxc_qnk_bvb} |
| Parent | Demand associated with the change request. This field is auto-populated. |
| Title | Title for the change request. |
| Description | Brief description of the change request and its impacts on the demand. |
| Business Justification | Reason for requesting the proposed change to the demand. |
| Work notes | Information about the change request. |
