An external facing Personal Data Rights (PDR) form configuration record holds the form-wide content that the requester sees across regions.

| Field | Description |

| External intake form title | Main title shown at the top of the public form. For example, <kbd class="ph userinput">Submit a personal data request</kbd>. |
| External intake form sub-title | Short description shown below the main title to give the requester context for the form. |
| Guidance - Request information | Help text shown on the page where the requester selects their request type. |
| Guidance - Contact information | Help text shown on the page where the requester provides their contact details. |
| Guidance - Final review | Help text shown on the review page before submission. |
| Privacy statement URL | Default link to your organization's privacy statement. |
| Legal terms URL | Default link to your organization's legal terms. |
| Terms of service URL | Default link to your organization's terms of service. |
