    Form fields on self-submission

    Field Description
    Request submitted by Indicator of who is filing the request. Select Self to submit a request for your own personal data.
    This request concerns Relationship of the requester to the organization, such as employee, vendor, or customer. The available options depend on the location selected.
    Note:
    The field denotes the data subject types that are configured for the selected location.
    Select your request type Privacy right to exercise for your data, such as the right to delete or the right to access. The available options depend on the data subject type and location selected.
    Request description Details of the request.
    Data subject details First name, last name, and email address of the requester. The email address is used to send updates about the request.

    Request submitted by Indicator of who is filing the request. Select Authorized agent to submit a request on behalf of an individual (data subject) whose personal data is involved.
    This request concerns Relationship of the data subject to the organization, such as employee, vendor, or customer. The available options depend on the country and state selected.
    Select your request type Privacy right being exercised for the data subject's data, such as the right to delete or the right to modify personal data. The available options depend on the selected data subject type and location.
    Request description Details of the request.
    Data subject details First name, last name, and email address of the data subject on whose behalf the request is being submitted.
    Agent details First name, last name, and email address of the authorized agent. The agent receives a verification email and subsequent updates about the request.
