Use the Create Connection and Credential form to establish a connection and authenticate your login credentials with an Everbridge instance.

| Field | Description |
|-|-|
| Connection Name | Name of the connection. Enter Everbridge Connection. |
| Connection URL | Everbridge API base address. |
| Organization ID | Organization ID of Everbridge. Note: You can get the organization ID from the Everbridge instance. |
| Incident Template ID | Unique ID of the Everbridge incident template. Note: You can get the ID from the Everbridge instance. |
| Credential Name | Name of the Everbridge credential. |
| Username | User name of the API user in Everbridge. |
| Password | Password to validate and establish the connection. |
