Use the Assessment Metric Type Record form to create an attestation template to create the attestations. Use the template form to create a record.

## Assessment Metric Type Record form {#attestation-template-reference__section_gtf_lps_ttb}

| Field | Description |
|-|-|
| Name | Name of the template. |
| Description | Information about the template. |
| Active | Option to enable the template. |
| State | State of the assessment. The available options are as follows: * Draft * Published {#attestation-template-reference__ul_ol5_szq_j5b}This field is auto-filled. |
| Metric categories | Category that represents a theme for evaluating assessable records in a given metric type. It displays the name of the template, description of the category, its weight, order, and so on.
