# Report an EMR issue with Care Team Operations for IT {#ariaid-title1}

Report any issue with your EMR system using the Report an EMR issue catalog item from the Care Team Operations portal.

Role required: sn_hco.care_team_member

## Prozedur

1. In the Care Team Portal, navigate to Create a requestRequest IT serviceReport an EMR issue.
2. Fill in the following fields.

   | Field | Description |
   |-|-|
   | Unit | The unit in which the EMR having an issue is located. |
   | Short description | A short summary of your issue. |
   | Describe your request in detail | Add a detailed description of the issue occurring with the EMR. Provide as much relevant information as possible. |
   | Urgency | The urgency of this issue. Options are: * 1 - High * 2 - Medium * 3 - Low {#hcls-cto-it-report-emr-issue__ul_d1z_bsy_4dc} |
   | Contact person | The associated contact for this request. |
   | Phone type | The phone type associated with the contact number. Options are: * Business * Mobile * Other {#hcls-cto-it-report-emr-issue__ul_w2j_bsy_4dc} |
   | Contact number | The phone number of the associated contact for this request. |

3. Select Submit.
