Request non-medical device issue biomed service using the Request other biomed service catalog item from the Care Team Portal.

## Prozedur

1. In the Care Team Portal, navigate to Create a requestRequest biomed serviceRequest other biomed service.
2. Fill in the following fields.

   | Unit | The unit associated with the biomed issue. |
   | Location | The location of the biomed issue. |
   | Short description | A short summary of your issue. |
   | Describe your request in detail | Add a detailed description of the biomed issue that has occurred. Provide as much relevant information as possible. |
   | Urgency | The urgency of this issue. Options are: * 1 - High * 2 - Medium * 3 - Low {#hcls-cto-biomed-request-other__ul_d1z_bsy_4dc} |
   | Contact person | The associated contact for this request. |
   | Phone type | The phone type associated with the contact number. Options are: * Business * Mobile * Other {#hcls-cto-biomed-request-other__ul_w2j_bsy_4dc} |
   | Contact number | The phone number of the associated contact for this request. |
